Sylvan Lake Triathlon Training Camp (the Camp
Participant Waiver and Release of Liability

I am applying for entry into the Sylvan Lake Triathlon Training Camp to be held   May 22-24, 2010. I am aware that participating in the camp involves accepting the risks, dangers and hazards associated with the camp. I realize that the activities undertaken will include, pool swimming, highway cycling, trail and pavement running. During these activities I will be exposed to, but not limited to, the following.

Natural and human made hazards such as uneven and rough road surfaces, loose gravel, steep hills and embankments, high winds, extreme and changeable weather conditions, motor vehicles, other bicycle and pedestrian traffic and marked and unmarked hazards, loose rocks, tree roots, wet and slippery footing and steep hills.   Acts, omissions and conduct, including NEGLIGENCE by or on behalf of the camp organizers, other participants, or users of the roadway and environs who may not have valid liability insurance.

I acknowledge that my application and any fee paid by me or on my behalf does not include or entitle me to the provision of any liability, medical or accident insurance benefits, and that it remains my responsibility to obtain valid or adequate liability, medical or accident insurance to cover my participation in the camp.   
I have trained and prepared sufficiently for this training camp. I fully realize that events of this nature could entail a great deal of risk to me, both for serious injury and even death. I also realize that this training camp could not be held unless I am willing to assume all consequences of these risks. Therefore I assume full responsibility to inform myself as to all dangers and risks and I sign this Participant Waiver and Release of Liability as a legally binding method of personally assuming all of these risks. In consideration of my entry I agree to execute this Accident Waiver and Release of Liability. I acknowledge that this training camp includes challenges that are an extreme test of a person's physical and mental limits and carries with it the potential for death, serious injury and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of athletes, equipment, vehicular traffic, actions of other people including, but not limited to, participants and/or producers of the camp, and lack of hydration. I hereby assume all of these risks of participating in this event. I realize that liability may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them or because of their possible liability without fault.

I certify that I am physically fit, have sufficiently trained for participation in the camp and have not been advised otherwise by a qualified medical person.
I acknowledge that this Accident Waiver and Release of Liability form will be used by the camp organizers and that it will govern my actions and responsibilities at said events.

In consideration of my application and permitting me to participate in this camp, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) Waive, Release and Discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me including my traveling to and from this event, The following person: Antony James Smith. (B) Indemnify and Hold Harmless the persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this event, whether caused by negligence of releases or otherwise.

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident or illness during this camp.

I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video or film likeness to be used for any legitimate purpose by the camp organizers and or assigns.
This Accident Waiver and Release of Liability shall be constructed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
